Email or hand deliver a copy of the completed registration documents to Admin@enochcreehockey.com

Or submit/upload through the online registration system.

 

Online Registration is only available if you have been:

  1. Registered to Enoch the previous year played OR
  2. Declared on your Parent Declaration Form that Player resides in Enoch OR
  3. Transfer is completed to play in Enoch for the year

Documents Required at Registration

  • 1 Completed ECHA Player Registration Form
  • 2 Completed Hockey Canada Medical Form
  • 3 Copy of Birth Certificate - Required for 1st Time Registrants Only
  • 4 Copy of Alberta Health Care Card - Required for 1st Time Registrants Only
  • 5 Copy of Indian Status or Metis Card - Required for 1st Time Registrants Only
